Request For Accessing NDIS Supports
To draw funds from NDIS, you need to request the NDIS access form and meet the eligibility criteria. As a responsible NDIS provider in Melbourne, Js Choice – Care and Support has been working on how we can improve the ways participants access the NDIS funds and make the most out of their plans.
This includes a series of steps:
- Contact a local NDIS support coordinator to ask for an NDIS Access Request Form (we can help you with that, simply call us anytime)
- Once you download and complete the NDIS Access Request form, you need to email it to enquiries@ndis.gov.au
- Get in touch with the NDIS (you’ll find all the information on the NDIS official site)
